Insurance & Medicare

We remind you that the responsibility for payment rests with the patient being treated or the parent/guardian. For office visits we expect payment at the time of that office visit by cash, check, or credit card (Visa/MasterCard/Discover). If you have insurance coverage, we will submit the necessary insurance forms to your insurance carrier. We remind you that most insurance contracts, including Medicare, involve deductibles and/or percentage allowances with the result that the entire bill is seldom covered in full. Should we receive any payment which exceeds your balance due, the excess will be refunded to you. Feel free to address any questions regarding your bill to our business office at 602.493.9361 during regular office hours.

Liability / Workman's Comp. / HMO's

If you are claiming workman's compensation, you must bring, from your employer or insurance company, a written acceptance of their financial responsibility.

In liability or contested workman's compensation acts, we remind you the responsibility for payment rests with the individual being treated, not necessarily the individual being sued. We expect you to pay your bill promptly. Once this is done, we will fulfill our responsibilities in providing your attorneys with necessary medical information.

HMO patients must have a referral at the time of their visit. Obtaining the referral is the patient's responsibility.
